What is the purpose of "Mailbox Assistant" -> "People Relevance" component ? 

Could someone please explain what PeopleRelevance is doing behind the scenes..

There is no official documents about what the specific purpose of "Mailbox Assistant" -> "People Relevance" component is.

Generally, the Microsoft Exchange Mailbox Assistant service performs background processing of mailboxes in the Exchange store.

The Assistants Infrastructure is part of the hosting application installation on servers that host the Mailbox server role. The Assistants Infrastructure is the component in Exchange 2010 that separates message processing business logic from the information store. This business logic includes the following features:

• E-mail retention
• Resource scheduling
• Automated out-of-office messages
• Automated calendaring features

To handle business logic processing, the Assistants Infrastructure makes use of event-based assistants known as Mailbox Assistants. The "People Relevance" component should be used to work for one of the message processing business logic above.
